Dependent
Verification Information and Health Care Audit
Beginning July 18, 2005, Delphi will be mailing
out to certain employees Dependent Verification Forms. This process is
necessary to verify the eligibility of dependents carried on health care
coverage under the Delphi Health Care Program for Hourly Employees. The
last hourly verification was conducted in 2002.
Dependent Verification letters will be sent to those hourly active employees,
retirees, surviving spouses and COBRA enrollees with dependents age 19-25,
stepchildren, principally supported children, guardianship and children
of domestic partners. Those receiving the Dependent Verification letters
will be required to verify eligibility for all of the dependents currently
listed on their health care coverage. If an employee does not have a dependent
that falls into one of the above dependent categories, they will not receive
Dependent Verification Information in the mail and will not be included
in the Health Care Dependent Audit.
Delphi will be utilizing a Voice Response Unit (VRU) and Internet capability
for Primary Enrollees to confirm eligibility and/or delete dependents.
The VRU, website and Call Center will open the week of July 18 and close
on August 5, 2005.
Once you have determined the eligibility of each dependent, please verify
it by using one of the following options:
- Access the Delphi National Benefit Center website (www.delphinbc.com)
and click on the ‘2005 Hourly Dependent Verification Audit’
link. You will need your Social Security Number and date of birth to access
your personal information on this secure website, or
- Dial 1-888-402-7249 and verify your dependent eligibility through the
use of an automated voice response system. This system will be available
24 hours a day, 7 days a week.
- Customer Service Representatives will be available to answer your questions
Monday through Friday from 7:30 a.m. to 4:30 p.m. Eastern Time beginning
July 18, 2005.
You must verify the continuing eligibility of each of your dependent(s)
listed using one of the methods above between July 18, 2005 and August
5, 2005 to avoid cancellation of their health care coverage. If you fail
to verify your dependent’s eligibility by August 5, 2005 Delphi
will cancel your dependents’ coverage effective August 31, 2005.
If you later request to reinstate health care coverage for previously
cancelled dependents, Delphi may require you to provide documentation
verifying that they initially and continuously met the Program’s
eligibility requirements. If you do not provide this documentation when
requested, your dependent(s) will not be re-enrolled. Additionally, Delphi
may request documentation verifying the eligibility of any enrolled dependent,
as necessary, to support continued enrollment.
Also this year, Delphi is verifying the accuracy of dependent Medicare
information on file. Please follow the instructions on the dependent verification
form to inform us if any of your dependents are currently enrolled for
Medicare coverage.
If you fall within the requirements for this audit, make certain that
you verify the qualifications of each dependent to avoid the chance of
having them removed from your health care coverage.
