Recent Vision Coverage Changes

Delphi and Cole Vision have agreed for Cole Vision to administer our Vision Insurance through a limited number of participating providers and doctors within each Zip Code area. In order for your insurance to be filed by the provider, you must utilize one of the agreed upon Providers as outlined by Cole Vision. For a list of providers call Cole Vision at 1-800-334-7591 and hold for an operator. Provide them with the your zip code or the zip code for the area that you wish to use. Or, you can go to their website at www.colemanagedvision.com and check for a provider. You will be asked for the plan number and that is 80141. Enter the plan number and your zip code for a list of providers.
